Water Shutoff Request Form

Registration Form:

  1. Complete and submit the form below.
  2. By completing the form below, you are providing Veer Towers UOA permission to share your contact and unit information with the plumber you have selected.
  3. You must contact the selected plumber within 48 hours after the form has been submitted to confirm the appointment date and time.
  4. Deposit in the amount of $250.00 must be made payable to Veer Towers UOA and submitted to the facilities office within 48 hours after submission of the form. If deposit is not received, your request will be cancelled.
  5. By selecting “Other Plumber”, you will be responsible to upload the plumbers license and insurance information prior to the form being submitted.

I Understand:

I have communicated to my tenants and property manager (if applicable), Veer Towers staff are allowed to enter the unit for an inspection once the work is completed.

I Understand:

The plumber must be onsite after 3pm, when the water is turned back on to ensure the installation was successful and no active leak(s) of any kind are present.

I Understand:

This form is just a request and does not guarantee the date and time for the work to be completed. It is my responsibility to schedule and confirm the date and time with the plumber that I have selected.

I Understand:

I am being notified and agree to take advantage of this shut off opportunity to conduct the voluntary replacement with the selected plumber.

I Understand:

If I am not going to be in the unit when the work is taking place, I must make arrangements with Concierge to have the key to my unit released to the contractors for the repairs in the unit. If this is not in place prior to the work taking place, the work will be rescheduled for the next available date. I will be responsible for any fees with the plumber.
